Physician Telehealth and Clinic Scheduling Processes

Immediate Access to Physician

Note: Patient will need their own smart phone // tablet for this

Step 1

Direct the patient to a QR code that will give him/her access to our partnership intake form or pediatric intake form

Patient will fill out the form in its entirety & then hit ‘Submit’

Step 2

Pull up Slack. Enter the following information about the patient, in the following order:

  • [Name]
  • [Phone number]
  • [Email]
  • [New patient / renewal patient]
  • [Peds patient?]
  • Ready for Doxy

 

The patient is now transferred over to the care of the clinic

Schedule Access to Physician

Clinic and Telehealth

Step 1

Direct the patient to a QR code that will give him/her access to our partnership intake form or pediatric intake form

Patient will fill out the form in its entirety & then hit ‘Submit.’ This form must be completed before leaving your care.

Step 2

Determine the day and time the patient would like to schedule their visit.

Clinic Hours:

  • 9 am – 12 pm
  • 1 pm – 6 pm

 

Step 3

Pull up Slack. Enter the following information about the patient, in the following order:

  • [Name]
  • [Phone number]
  • [Email]
  • [New patient / renewal patient]
  • [Peds patient?]
  • [Virtual or clinic]
  • [Requested day and time of appointment]
If another patient is already scheduled for the time your patient is wanting, a THC2 representative will call the patient and make adjustments at a later date.

The patient is now transferred over to the care of the clinic

More often than not, your patient will have their own tools to access the intake form via QR code or link. However, if you want to ensure a patient is capable of having their physician’s recommendation completed on the spot, it’s suggested to have a business-owned screen available to them.
